  • A Health Home is a Medicaid program where you’re assigned a care manager through the system.

    It’s valuable support, but it’s structured around caseloads, eligibility, and what the program allows. Care managers are often supporting many clients at once and working within system limitations.

The first thing we do is stop anything from getting worse. We identify what’s urgent, what can’t wait, and what will fall apart if nobody handles it today. Everything else comes after that.

Before we build anything, we slow it down. Not just for the facts, but for what’s not being said. Where the confusion is. What hasn’t been explained. What someone’s carrying but hasn’t fully named. We make sure everything is clear before we move forward.

Saraii is Congolese, French, and American, born in France and shaped by experiences that taught her early what it means to move through systems that were not built for you. That understanding is reflected in everything she does. It is not just professional, it is personal.
